 Plan Options

1) The individual user plan.* This plan allows clinicians to assign up to 10 tests per month for a fee of $29.99 USD per month (~$3.00 per test). Click here to register for the individual user plan.

2) The power user plan.* This plan allows clinicians to assign up to 40 tests per month for a fee of $59.99 USD per month (~1.50 per test).

Once you have placed your order, you wil be sent by email your user ID and password to access the PDQ-4 clinician portal. It will take between 24-48 hours to activate your account. Please make sure to read theterms and conditionsbefore registering.

*You will be billed monthly on a reoccurring basis until you cancel you account.

Here is what we offer

Have your clients complete the PDQ-4 personaliity disorder screening test including the PDQ+ clinical significance scale questions through our site.

Using your own personal web space, you can assign clients a specific ID number which they can use to complete the PDQ-4 questionnaire in your practice or at home. Using your web space you are then able to review your clients' responses and download summary information that may be included in your clinical reports.

Summary and detailed reports of each client's responses are available and the raw data on the responses to each question can be downloaded in csv. (spreadsheet) format  for research purposes.
